How blended case management works

Healing Bridges assists people of all ages with a variety of behavioral health needs by building a strong network of community resources and services. Our blended case management program is a free-of-charge service that links clients to needed resources within the community and navigates complex systems to help individuals and families gain access to those local support services.

The role of a blended case manager (BCM) is to provide traditional and nontraditional hours of assistance to individuals diagnosed with a mental illness or emotional disorder. BCMs are dedicated to helping clients overcome challenges and connect people to the resources that they need.

What we help with

Our blended case managers assist in the following sectors:

  • Basic living needs (i.e. employment, housing, food, medical care and recreation and on-site crisis intervention, etc.)
  • Daily living activities (i.e. hygiene, grocery shopping, preparing meals, safety skills, etc.)
  • Network of socialization resources for support
  • Community services and resource linkages
  • Public transportation linkages for independence needs
  • Medical treatment for mental health and substance abuse problems
  • Service delivery monitoring
  • Client outreach to increase communication between the treatment team and the receiver
  • Education and vocational rehabilitation services
  • Income and benefits program solutions

Who is eligible?

Any person living in Washington County with a mental health diagnosis that is in need of high level mental health service resource linkages is eligible for BCM. The level of case management service is determined by the intensity of each person’s unique situation.

Healing Bridges’ BCM services are provided with no cost. Beacon Value Behavioral Health or the County Behavioral Office funds the cost of BCM services for those who meet the criteria.

How to get started

Please call the BCM Supervisor at 724-225-6940 x 157 and request blended case management services. Once our services are requested, the Center for Community Resources (CCR) and Healing Bridges will determine eligibility to start services, and treatment will be implemented on an individual basis. If the individual meets the proper criteria, then a blended case manager will be assigned. You can also find us on findhelp.org to get connected with a team member for guidance on the intake process.

If you are already an active client at Healing Bridges in a different program, you simply need to fill out the mental health BCM referral form located on the website and email it to the BCM supervisor. For help filling out the referral form, please call 724-225-6940 for assistance.
